What is the proper way to re-level pavers in a walkway that have settled??

Also, moss has a tendancy to appear between the pavers. What's the best way to prevent that?

For leveling, the best way is to pull up the pavers and use a rake/shovel to flatten out the base. Ideally you would rent a compactor, which vibrates the ground and packs it down. (This is a gas powered machine, about the size of a lawn mower, which you can rent at most tool rental centers). If the pavers are along the side of your house and you have a basement or crawlspace, be sure to install them with a slight slope AWAY from your house, to ensure that rain water runs away from your house.

The moss grows because of moisture presence. The pavers are likely sitting right on top of soil, which holds moisture. If you include a layer of coarse sand a few inches thick underneath the pavers, this will allow for good drainage and reduce the moss growth.
